Form 7A

Plaintiff's Claim. The form that starts a Small Claims Court lawsuit — for money or property up to $50,000, excluding interest and costs.

Form 15A

Notice of Motion and Supporting Affidavit. Asking the court for something along the way — most commonly, to set aside a default judgment or noting in default.

Summons to Witness. Compels a witness to attend trial and bring documents — served personally, with attendance money, well before the trial date.

Notice of Garnishment. The workhorse of collection — intercepts the debtor's wages or bank account to pay your judgment.

Writ of Seizure and Sale of Personal Property. Directs the court enforcement office to seize and sell the debtor's personal property — vehicles, equipment, inventory.
